Why this applies to Ciclesonide

Ciclesonide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Aegeline interacts with.

Aegeline inhibits cytochrome P450 3A4 (CYP3A4) activity in vitro. So far, this interaction has not been reported in humans. Theoretically, aegeline may increase levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others. Use aegeline cautiously or avoid in patients taking these drugs.

Here's the gist for your inhaled ciclesonide (Alvesco or Omnaris). Aegeline has been shown in test-tube studies to slow down CYP3A4, an enzyme your body uses to break down many medicines. Ciclesonide does rely on that enzyme to some degree, so in theory, blocking it could let a bit more of the active steroid linger in your system.

The good news: this is only laboratory evidence and has never been seen in real people, and the rating is minor. Inhaled doses are also quite low to begin with. Even so, I'd skip aegeline entirely, since it's been tied to serious liver injury and offers no proven benefit. If you're considering it, please run it past your pharmacist or doctor first.

Aegeline is a compound originally identified in the bael tree (Aegle marmelos) that gained attention because it was added to some weight-loss and workout supplements. It is best known for being linked to serious liver injury in a product called OxyELITE Pro, and U.S. regulators warned against products containing it. There is no good evidence it safely helps with weight loss, and it should generally be avoided.

What Is Ciclesonide Oral Inhalation Used For?

Brand names: Omnaris, Alvesco, Zetonna

Alvesco (ciclesonide) inhaler is approved for the maintenance treatment of asthma as a preventive therapy in adults and children 12 years of age and older. It is taken regularly to reduce airway inflammation and help prevent asthma symptoms — not to relieve a sudden attack.

Alvesco is not a rescue inhaler and should not be used to treat sudden wheezing or acute asthma episodes. It is also not approved for children under 12.
