Aspirin, Caffeine, Codeine Phosphate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Aegeline interacts with.

Aegeline inhibits cytochrome P450 3A4 (CYP3A4) activity in vitro. So far, this interaction has not been reported in humans. Theoretically, aegeline may increase levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others. Use aegeline cautiously or avoid in patients taking these drugs.

Here's the thing to know about this combo pill: the codeine part is actually a prodrug. Your body has to switch it on (mostly through a different enzyme, CYP2D6) to give pain relief. The caffeine in it leans on the enzyme aegeline is said to block, CYP3A4. So in theory, if aegeline slowed that pathway, caffeine could linger a bit longer and leave you feeling jittery, with a racing heart or trouble sleeping. Aspirin and the pain-relieving action of codeine aren't really driven by CYP3A4, so they shouldn't change much here.

Honestly, this concern comes only from lab dish studies, not real people, so it's very theoretical. My bigger worry is aegeline itself, since it's been tied to serious liver damage and regulators warned against it. I'd avoid it and check with your pharmacist.

Aegeline is a compound originally identified in the bael tree (Aegle marmelos) that gained attention because it was added to some weight-loss and workout supplements. It is best known for being linked to serious liver injury in a product called OxyELITE Pro, and U.S. regulators warned against products containing it. There is no good evidence it safely helps with weight loss, and it should generally be avoided.
