Dextroamphetamine (patch) falls under Cytochrome P450 2D6 (Cyp2D6) Substrates — the group Puravive interacts with.

Puravive contains Panax ginseng. Some research suggests that Panax ginseng might slow down the activity of an enzyme called CYP2D6 by about 6%. This enzyme plays a role in breaking down drugs in our system. One study (https://pubmed.ncbi.nlm.nih.gov/32557706/) found that Panax ginseng slows down the breakdown of dextromethorphan, a drug that is processed by CYP2D6, but only to a small extent. It is important to note that not all studies agree on these effects, and some suggest that Panax ginseng might not have a significant impact on CYP2D6 at all. Nevertheless, it's important to be cautious if you're taking medications that are processed by CYP2D6 and consider discussing it with your healthcare provider.

Here's the short version: Puravive contains Panax ginseng, and some research hints it might slightly slow down CYP2D6, one of the liver enzymes that helps clear your dextroamphetamine patch (Xelstrym). Dextroamphetamine is an active drug, not a prodrug, so if that enzyme runs a bit slower, in theory a little more medication could linger in your system.

Honestly though, the effect seen in studies was tiny (roughly 6%), the evidence is weak, and some studies show no real impact at all. So I wouldn't lose sleep over it. Still, if you notice more of the usual amphetamine effects, like a racing heart, jitteriness, trouble sleeping, or a poor appetite, mention it to your pharmacist or doctor before making any changes.

Here's the gist for your Xelstrym patch: Puravive contains Panax ginseng, and some studies suggest ginseng can have a mild stimulant-like effect of its own. Since your patch already delivers dextroamphetamine, a stimulant, combining the two could in theory nudge those stimulant effects a little higher. Think faster heartbeat, jitteriness, trouble sleeping, or feeling more wound up than usual.

Honestly though, the evidence here is thin and this interaction is rated as unlikely and minor, so I wouldn't lose sleep over it. Dextroamphetamine isn't a prodrug, so ginseng isn't changing how it activates. Just keep an eye out for extra restlessness or a racing heart, and check with your pharmacist or doctor before starting Puravive.
