Benzhydrocodone, Acetaminophen falls under Cytochrome P450 2D6 (Cyp2D6) Substrates — the group Puravive interacts with.

Puravive contains Panax ginseng. Some research suggests that Panax ginseng might slow down the activity of an enzyme called CYP2D6 by about 6%. This enzyme plays a role in breaking down drugs in our system. One study (https://pubmed.ncbi.nlm.nih.gov/32557706/) found that Panax ginseng slows down the breakdown of dextromethorphan, a drug that is processed by CYP2D6, but only to a small extent. It is important to note that not all studies agree on these effects, and some suggest that Panax ginseng might not have a significant impact on CYP2D6 at all. Nevertheless, it's important to be cautious if you're taking medications that are processed by CYP2D6 and consider discussing it with your healthcare provider.

Here's the wrinkle with Apadaz: the benzhydrocodone part is actually a prodrug. Your body has to convert it into hydrocodone before it does much, and then an enzyme called CYP2D6 turns some of that into a stronger painkiller (hydromorphone). Puravive contains Panax ginseng, which might slow CYP2D6 down a little. If that enzyme works slower, you could get slightly less of the active pain relief on the back end.

Honestly, the evidence here is weak and the effect is tiny (one study saw roughly a 6% change, and not all studies even agree). For most people this won't be noticeable. Still, opioids deserve respect, so check with your pharmacist or doctor before pairing them, especially since Puravive itself isn't well studied.

Benzhydrocodone, Acetaminophen also falls under Cytochrome P450 3A4 (Cyp3A4) Substrates.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's something worth knowing about Apadaz: the benzhydrocodone part is actually a prodrug. It has to be converted in your body into hydrocodone, the piece that relieves pain. Puravive contains Panax ginseng, which can nudge the CYP3A4 enzyme either way (the research goes in both directions, so nobody can say for sure).

If ginseng speeds that enzyme up, you might clear the active drug faster and get less pain relief. If it slows it down, the opioid could build up, which can mean more drowsiness, dizziness, constipation, or slowed breathing. The evidence here is weak and the effect is rated minor, so don't panic. Still, since opioids are nothing to gamble with, run this combo by your pharmacist or doctor first.

What Is Benzhydrocodone and Acetaminophen Used For?

Brand name: Apadaz

Apadaz is indicated for the short-term management of acute pain — specifically pain severe enough to require an opioid analgesic and for which other treatments are inadequate. It is approved for use for no more than 14 days.

Apadaz is reserved for patients who have not been able to tolerate non-opioid pain relievers, or for whom those options have not provided adequate pain relief. It should not be used long-term unless pain remains severe and alternative treatments continue to be insufficient.
