Here's the short version: the concern with pairing acetyl-L-carnitine and your dextroamphetamine patch (Xelstrym) is that both may nudge serotonin activity in the brain a little higher. Dextroamphetamine isn't a prodrug, so this isn't about how it's broken down. It's simply that stacking two things that raise serotonin could, in theory, add up.

I want to be honest about how thin the evidence is here. This flag comes from animal studies only, so it's largely theoretical. Still, if you notice things like a racing heart, agitation, sweating, shakiness, or feeling unusually restless, take that seriously and check in with your doctor or pharmacist. It's an easy conversation to have before you start the carnitine, and I'd never suggest changing your patch on your own.
