Here's the gist for Aristada (aripiprazole lauroxil), the long-acting injection you get for schizophrenia or bipolar I. Acetyl-L-carnitine may gently nudge up serotonin activity in the brain. Since aripiprazole also touches serotonin pathways, the worry is that combining the two could, in theory, raise the small risk of serotonin-related effects like agitation, restlessness, shakiness, sweating, a fast heartbeat, or confusion.

I want to be honest about the strength here: this is based only on animal and lab studies, not real people, so the actual risk looks low. Still, because your aripiprazole stays in your body for weeks after each shot, it's worth a quick heads-up to your pharmacist or prescriber before you start carnitine, especially if you notice any new jitteriness or unusual symptoms.
