Here's the simple version: Trimipramine is a tricyclic antidepressant that works by raising serotonin and other brain chemicals. Animal studies suggest that acetyl-L-carnitine may also nudge serotonin levels up a bit. So in theory, taking both together could add to that serotonin effect and slightly raise the chance of serotonin-related side effects, like agitation, a racing heart, sweating, shivering, or tremor.

I want to be honest about how weak this evidence is. It comes from lab and animal work, not from real people, so for most folks this is a low-level, theoretical concern rather than something that's been clearly shown. Trimipramine isn't a prodrug, so it doesn't flip the direction here. Still, before you add acetyl-L-carnitine, run it by your pharmacist or doctor so they can keep an eye on you.

What Is Trimipramine Used For?

Trimipramine capsules are indicated for the relief of symptoms of depression. Endogenous depression — depression that arises from within, rather than from an obvious outside cause — is more likely to respond to treatment than other types.

In clinical studies, trimipramine worked as well as imipramine in hospitalized patients with depression, and was comparable to amitriptyline in less severely depressed outpatients.
