Here's the thinking on pairing acetyl-L-carnitine with selegiline. Selegiline is an MAO-B inhibitor for Parkinson's, and one of its quirks is that it can raise activity of brain chemicals like serotonin, especially at higher doses or with the patch (Emsam). Animal studies suggest acetyl-L-carnitine may nudge serotonin levels up a bit too. Stacked together, the theoretical worry is too much serotonin activity, which can cause restlessness, sweating, a racing heart, shivering, muscle twitching, or confusion (signs of serotonin syndrome).

I want to be honest: this is based on lab and animal data, not solid human cases, so it's a low-likelihood concern. Still, because selegiline already affects these pathways, please run it by your pharmacist or doctor before adding the supplement.

Selegiline is used as an add-on treatment for people with Parkinson's disease who are already taking levodopa/carbidopa but are finding that the medication's effects are wearing off or becoming less reliable. It is not used alone — it only works as part of a combination approach.
