Here's the situation with alosetron (Lotronex) and acetyl-L-carnitine. Alosetron actually works in the serotonin system, but it does the opposite of what carnitine might do: it blocks certain serotonin (5-HT3) receptors to slow down your gut and ease the diarrhea, urgency, and cramping of IBS. The concern on file is that acetyl-L-carnitine may nudge serotonin levels higher, which in theory could add to serotonin activity in the body.

I want to be honest about the strength of this: it comes only from animal and lab research, so it's theoretical, not something seen clearly in people. The realistic worst case would be serotonin-type symptoms like agitation, sweating, a racing heart, or shivering, but that's unlikely. Still, before you pair these two, just run it by your pharmacist or doctor.

About Alosetron

What Is Alosetron Used For?

Brand name: Lotronex

Alosetron is used only in women with severe diarrhea-predominant irritable bowel syndrome (IBS). It is for people whose symptoms have lasted 6 months or longer, whose doctor has ruled out other gut problems, and who have not improved with conventional therapy.

IBS counts as severe when it includes diarrhea plus one or more of: frequent and severe belly pain or discomfort, frequent urgency or accidents, or limits on daily activities. The same use applies to the alosetron, alosetron hydrochloride and Lotronex tablets. Studies have not adequately confirmed benefit in men.
