Dextroamphetamine (patch) falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Berberine interacts with.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP2D6.
In vitro research and preliminary clinical evidence show that berberine can inhibit CYP2D6. Theoretically, use of berberine with drugs metabolized by CYP2D6 might increase drug levels and increase the risk of adverse effects.

Here's the short version for your Xelstrym patch. Dextroamphetamine is partly cleared by a liver enzyme called CYP2D6, and berberine may slow that enzyme down a bit. When the enzyme works slower, more of the medicine can hang around in your body, so its effects (and side effects) may feel stronger.

In real life with dextroamphetamine, that could look like a faster or pounding heartbeat, higher blood pressure, trouble sleeping, jitteriness, reduced appetite, or feeling wired. Keep in mind this is mostly theoretical and based on lab work plus limited human data, so it may not happen to everyone. Still, since a stimulant already nudges heart rate and blood pressure, it's worth a quick chat with your pharmacist or doctor before combining them.
