Why this applies to Dextroamphetamine (patch)
Dextroamphetamine (patch) falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Goldenseal interacts with.
Clinical and in vitro research shows that goldenseal can significantly inhibit CYP2D6 enzymes, potentially increasing levels of drugs metabolized by CYP2D6.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the short version for your Xelstrym patch: goldenseal can slow down a liver enzyme called CYP2D6, which is one of the routes your body uses to clear dextroamphetamine. When that pathway gets partly blocked, the medicine can hang around longer and build up a bit. Dextroamphetamine works as-is (it's not a prodrug that needs activating), so higher levels usually mean a stronger, more amplified effect.
In practice that could show up as a faster or pounding heartbeat, higher blood pressure, jitteriness, trouble sleeping, or reduced appetite. The evidence here is fair, not rock-solid, so I wouldn't panic, but I also wouldn't pair them on your own. Please check with your pharmacist or prescriber before adding goldenseal.
