Metoprolol falls under ANTIHYPERTENSIVE DRUGS — the group Goldenseal interacts with.

Theoretically, goldenseal might increase the risk of hypotension when taken with antihypertensive drugs.
Goldenseal contains berberine. Animal research shows that berberine can have hypotensive effects. Also, an analysis of clinical research shows that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone. However, this effect has not been reported with goldenseal.

Here's the gist with goldenseal and your metoprolol. Goldenseal contains berberine, and the thinking is that berberine may have a mild blood pressure lowering effect of its own. Since metoprolol already works to slow your heart rate and lower your blood pressure, stacking the two could theoretically push things a little too far.

If that happened, you might notice signs of your numbers dipping low: lightheadedness, dizziness when you stand up, unusual tiredness, or a slow pulse. I want to be honest though, this concern is theoretical and based mostly on lab and animal work, not on real cases with goldenseal itself. Still, before adding it, it's worth a quick chat with your pharmacist or doctor, especially since they can check your blood pressure.

Here's the simple version: your body relies on an enzyme called CYP2D6 to clear metoprolol out of your system. Goldenseal can slow that enzyme down. When the cleanup crew works slower, more metoprolol can linger in your blood than usual.

For you, that could mean a stronger effect than expected, so watch for a heart rate that runs too low, dizziness or lightheadedness (especially when standing up), unusual tiredness, or feeling faint. The evidence here is real but moderate, based on some lab and lower-quality human research, so this isn't a guarantee it will happen to everyone. Still, please don't start or stop anything on your own. Check in with your pharmacist or doctor first, and they may want to keep an eye on your pulse and blood pressure.

About Metoprolol

What Is Metoprolol Used For?

Brand names: Lopressor, Toprol XL

Metoprolol is used to treat several heart and blood pressure conditions. Metoprolol succinate extended-release tablets (including Toprol-XL) are approved for high blood pressure, angina pectoris (chest pain from the heart), and heart failure. Metoprolol tartrate tablets (including Lopressor) are approved for high blood pressure, angina, and — when used alongside intravenous metoprolol — to reduce the risk of cardiovascular death after a definite or suspected heart attack.

Metoprolol tartrate injection (intravenous form) is used in hemodynamically stable patients with a definite or suspected acute heart attack to reduce cardiovascular death, and is followed by oral metoprolol therapy. Lowering blood pressure with metoprolol reduces the risk of strokes and heart attacks.
