Why this applies to Triamcinolone

Triamcinolone falls under P-GLYCOPROTEIN SUBSTRATES — the group Goldenseal interacts with.

Theoretically, goldenseal might increase or decrease serum levels of P-glycoprotein (P-gp) substrates.
There is conflicting evidence about the effect of goldenseal on P-gp. In vitro research suggests that berberine, a constituent of goldenseal, modestly inhibits P-gp efflux. Other evidence suggests that berberine induces P-gp. In healthy volunteers, goldenseal modestly increases peak levels of the P-gp substrate digoxin by about 14%. However, it does not seem to affect other pharmacokinetic parameters such as area under the curve (AUC). This suggests that goldenseal is not a potent inhibitor of P-gp-mediated drug efflux. Until more is known, goldenseal should be used cautiously with P-gp substrates.

Here's the gist if you're using triamcinolone (a steroid for things like arthritis flares, skin conditions, allergies, or asthma) alongside goldenseal. Triamcinolone is moved around the body partly by a transporter called P-glycoprotein, and goldenseal's main compound, berberine, can nudge that transporter. The catch is the research pulls in both directions: some lab work says it slightly blocks the transporter, other work says it speeds it up.

In real people, the effect has been small (about a 14% bump in peak levels of a similar drug, with no real change overall), so this is mostly theoretical for triamcinolone. Still, if you take both, it's worth a quick chat with your pharmacist or doctor, especially before starting concentrated goldenseal supplements rather than occasional culinary amounts.

Triamcinolone is available in many forms — injections, creams, ointments, nasal sprays, dental pastes, and eye injections — each approved for specific uses. Injectable forms given into muscle are used when oral steroids aren't an option, covering severe allergies (like asthma flares, severe skin reactions, drug reactions, and serum sickness), serious skin diseases, certain endocrine (hormonal) disorders, and more.

Injections given directly into a joint (intra-articular) or soft tissue help with conditions like rheumatoid arthritis, gout flares, bursitis, and tendon inflammation. Injections into skin lesions (intralesional) treat conditions such as keloids, alopecia areata, and psoriatic plaques. Triesence (an ophthalmic injection) is specifically indicated for eye conditions like uveitis and temporal arteritis, and for visualization during eye surgery. Topical creams, ointments, lotions, and sprays relieve itching and inflammation from corticosteroid-responsive skin conditions. Dental paste forms (Oralone, Kourzeq) treat painful mouth sores from injury. Nasal sprays temporarily relieve hay fever and upper respiratory allergy symptoms such as runny nose, sneezing, and congestion.
