Hydrochlorothiazide falls under ANTIHYPERTENSIVE DRUGS — the group Goldenseal interacts with.

Theoretically, goldenseal might increase the risk of hypotension when taken with antihypertensive drugs.
Goldenseal contains berberine. Animal research shows that berberine can have hypotensive effects. Also, an analysis of clinical research shows that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone. However, this effect has not been reported with goldenseal.

Here's the gist with goldenseal and your water pill, hydrochlorothiazide. Goldenseal contains berberine, and in animal studies (plus some human work with other blood pressure meds) berberine seems to gently nudge blood pressure downward. Since hydrochlorothiazide already lowers blood pressure by helping your kidneys flush out extra salt and water, stacking the two could add up and push your pressure lower than intended.

If that happened, you might feel dizzy, lightheaded, or a bit faint, especially when standing up quickly. I want to be honest, though: this concern is theoretical and based mostly on lab and animal data, not real reports with goldenseal itself, so it's a maybe, not a definite. Don't change anything on your own. Just give your pharmacist or doctor a heads-up before combining them.

What Is Hydrochlorothiazide Used For?

Brand names: Esidrix, Hydro, Microzide, HydroDiuril, Hydro-D, Oretic, Aquazide H, Aquazide, HCTZ

Hydrochlorothiazide treats high blood pressure (hypertension) in adults and children, either alone or combined with other blood pressure medicines. Lowering blood pressure reduces the risk of strokes and heart attacks.

It also treats edema (fluid buildup) linked to congestive heart failure, hepatic cirrhosis (liver scarring), and kidney conditions including nephrotic syndrome. Hydrochlorothiazide tablets are additionally indicated for edema caused by corticosteroid or estrogen therapy, and edema from acute glomerulonephritis or chronic renal failure. Inzirqo (the oral suspension) is specifically approved for both hypertension and edema in adult and pediatric patients.
