Here's the situation with goldenseal and your formoterol inhaler. Goldenseal contains berberine, which can slow down a liver enzyme called CYP2D6. Formoterol is partly cleared by this same enzyme, so blocking it could let more of the drug linger in your system. Formoterol isn't a prodrug, meaning it works as-is, so higher levels generally mean stronger effects.

In real life, that might show up as a faster or pounding heartbeat, jitteriness, shakiness, or a headache. The evidence here is moderate, not rock-solid, but it's worth taking seriously since heart-related side effects can be uncomfortable. Please don't stop either one on your own. Just check with your pharmacist or doctor before adding goldenseal, and mention any new palpitations or tremor.

Theoretically, goldenseal might increase serum levels of drugs metabolized by CYP2C9.
In vitro research shows that goldenseal root extract can modestly inhibit CYP2C9. This effect may be due to its alkaloid constituents, hydrastine and berberine. However, this effect has not been reported in humans.

Here's the short version on mixing goldenseal with your formoterol inhaler. In test-tube studies, goldenseal can mildly slow down a liver enzyme called CYP2C9. If that same effect happened in your body, a drug cleared by that enzyme could build up a bit and cause stronger effects. For formoterol, that might look like a faster or pounding heartbeat, jitteriness, shakiness (tremor), or a headache.

That said, I want to be honest about the evidence. This is purely theoretical, seen only in lab dishes and never confirmed in real people, so for most folks it's unlikely to cause trouble. Still, since concentrated goldenseal supplements are stronger than any culinary amount, it's worth a quick chat with your pharmacist or doctor before you start it. Please don't change your inhaler on your own.

What Is Formoterol Oral Inhalation Used For?

Brand names: Foradil Aerolizer, Atimos Modulite, Oxis Turbohaler, Perforomist, Foradil Certihaler

Formoterol fumarate inhalation solution is used for the long-term maintenance treatment of airway narrowing (bronchoconstriction) in adults with COPD, including chronic bronchitis and emphysema. It is taken twice daily — morning and evening — as a regular, ongoing treatment.

Important limits: This medication is not for relieving sudden COPD flare-ups or breathlessness in the moment, and it is not approved to treat asthma. A separate short-acting inhaler should always be on hand for sudden symptoms.
