Fentanyl falls under CNS DEPRESSANTS — the group Goldenseal interacts with.

Theoretically, goldenseal might increase the sedative effects of CNS depressants.
Goldenseal contains berberine. Animal research shows that berberine can have sedative effects. However, this effect has not been reported in humans.

Here's the simple version: goldenseal contains a compound called berberine, and in animal studies berberine seemed to have a mild calming or sedating effect. Fentanyl is already a powerful opioid that slows the brain and breathing, so the worry is that stacking these two could add up and make you more drowsy than expected.

I want to be honest about how solid this is. This concern is theoretical, based on lab and animal work, and that extra sedation has not actually been reported in people. Still, fentanyl is potent and even a little extra drowsiness, confusion, or slowed breathing matters a lot. If you're on fentanyl, please check with your pharmacist or doctor before adding goldenseal, and watch for unusual sleepiness.

Clinical and in vitro research shows that goldenseal can significantly inhibit CYP2D6 enzymes, potentially increasing levels of drugs metabolized by CYP2D6.

Here's the honest picture with Goldenseal and fentanyl. Goldenseal can slow down a liver enzyme called CYP2D6, and the interaction on file points to that pathway. The thing is, fentanyl is mainly broken down by a different enzyme, CYP3A4, not CYP2D6. So this particular interaction is likely to have a small effect on fentanyl, if any.

That said, fentanyl is a powerful opioid, and even minor shifts deserve respect. If levels did creep up, you might notice more drowsiness, slowed breathing, confusion, or constipation. Because the stakes with fentanyl are high, please don't add goldenseal without first checking with your pharmacist or doctor so they can look at your full medication list.

Fentanyl also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to break down fentanyl and keep its level in a safe range. Goldenseal appears to slow that enzyme down. When the cleanup process is slowed, more fentanyl can linger in your system than expected.

With a powerful opioid like fentanyl, that's a real concern. Even a modest rise in blood levels can mean more drowsiness, confusion, very slow or shallow breathing, and dangerously low oxygen. Because fentanyl has so little room for error, this is not a combo I'd take lightly. The evidence here is moderate, not certain, but I'd play it safe. Please check with your doctor or pharmacist before pairing goldenseal with fentanyl, and never adjust your dose on your own.

What Is Fentanyl Used For?

Brand names: Sublimaze, Duragesic, Fentora, Lazanda, Abstral, Subsys

Fentanyl transdermal system is prescribed for severe, persistent pain that cannot be controlled with other pain medicines, including shorter-acting opioids. It is only for people who are already opioid-tolerant — meaning they've been taking a significant daily opioid dose for at least a week.

It is not for short-term pain, post-surgical pain, mild pain, or use on an as-needed basis. Because of serious risks including addiction, overdose, and death, it is reserved for situations where other treatment options have failed or are not appropriate.
