Why this applies to Diclofenac Sodium

Diclofenac Sodium falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Goldenseal interacts with.

Theoretically, goldenseal might increase serum levels of drugs metabolized by CYP2C9.
In vitro research shows that goldenseal root extract can modestly inhibit CYP2C9. This effect may be due to its alkaloid constituents, hydrastine and berberine. However, this effect has not been reported in humans.

Here's the simple version. Your body clears diclofenac using a liver enzyme called CYP2C9. In lab dishes, goldenseal (thanks to its alkaloids berberine and hydrastine) appears to slow that enzyme down a bit. If that happened in people, diclofenac could build up more than expected, which might mean more of this drug's usual side effects: stomach upset, heartburn, a higher chance of stomach irritation or bleeding, fluid retention, or raised blood pressure.

The key point is that this is only seen in test-tube studies, not in actual humans, so it stays theoretical for now. I wouldn't lose sleep over it, but since both can be hard on the gut, it's worth a quick chat with your pharmacist or doctor before combining concentrated goldenseal supplements with diclofenac.

Here's the basic idea with goldenseal and diclofenac. Diclofenac is an NSAID, and like other drugs in that family it can already irritate the stomach lining and make it a little easier to bleed. Goldenseal contains a compound called berberine, and in test tube and animal studies it appears to make platelets (the cells that help your blood clot) less sticky. Put those two together and the concern is a slightly higher chance of bleeding or bruising.

I want to be honest about how soft this evidence is. The blood-thinning effect of goldenseal has only been seen in the lab, not in actual people, so this is more theoretical than proven. Still, if you're on diclofenac and want to use goldenseal, it's worth a quick chat with your pharmacist or doctor first, and watch for things like unusual bruising, black or tarry stools, or stomach pain.

Diclofenac sodium topical gel is used to treat actinic keratoses (AK) — rough, scaly skin patches that form after years of sun exposure. These patches are considered precancerous, meaning they can develop into skin cancer if left untreated.

This is a topical gel, meaning it is applied directly to the skin over the affected area rather than taken by mouth. It belongs to the NSAID (nonsteroidal anti-inflammatory drug) family.
