Goldenseal contains berberine. Animal research shows that berberine can have sedative effects. However, this effect has not been reported in humans.

This combination pill contains codeine, which already has a calming, drowsy side to it. The concern with goldenseal is that one of its plant compounds, berberine, showed mild sedative effects in animal studies. The thinking is that goldenseal could add to the sleepiness this medicine can cause, so you might feel more tired, foggy, or slowed down than usual.
Here's the honest part: this is a theoretical worry. It hasn't actually been seen in people, only in lab animals, so I wouldn't lose sleep over it. Still, since codeine can make you drowsy on its own, just be a little cautious until you know how you feel, and check with your pharmacist or doctor before pairing the two.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Clinical and in vitro research shows that goldenseal can significantly inhibit CYP2D6 enzymes, potentially increasing levels of drugs metabolized by CYP2D6.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's a wrinkle worth knowing about this combo pill. The codeine in it is actually a prodrug, meaning it doesn't do much on its own. Your body has to convert it into morphine using an enzyme called CYP2D6, and that's what gives you the pain relief. Goldenseal can block that same enzyme.
So in this case, goldenseal may actually leave you with less pain relief from the codeine, not more, because less of it gets turned into the active form. The acetaminophen, caffeine, and salicylamide parts aren't really affected this way. The evidence here is moderate, but it's a real concern. If you take this medicine for ongoing pain, please check with your pharmacist or doctor before adding goldenseal so they can keep you comfortable.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

This combo product has a lot going on, but the piece worth watching is the codeine. Goldenseal tends to slow down a liver enzyme called CYP3A4. Codeine relies partly on that enzyme as one of its breakdown routes, so blocking it can shift how your body handles the drug and may let more of it (and its byproducts) hang around longer. In practice that could mean stronger sedation, drowsiness, constipation, or feeling lightheaded.
The caffeine in this product can also linger a bit longer with goldenseal on board, so you might feel more jittery or have trouble sleeping. The evidence here is real but modest, so it is not a guaranteed problem. Still, before pairing goldenseal supplements with this medicine, it is worth a quick check with your pharmacist or doctor.

CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES

In vitro research shows that goldenseal root extract can inhibit the activity of CYP2E1. However, this effect has not been reported in humans.

This combination product mixes acetaminophen (Tylenol), caffeine, codeine, and salicylamide. The piece that touches goldenseal here is the acetaminophen, which is partly handled by a liver enzyme called CYP2E1. In lab dish studies (not in real people), goldenseal's berberine seemed to slow that enzyme down. If that happened in the body, it could shift how acetaminophen is broken down.
Here's the honest part: this is theoretical and has never actually been shown in humans, so I wouldn't lose sleep over it. The bigger thing I'd watch with this medicine is the codeine side, which can cause drowsiness, constipation, and breathing slowdown. If you take goldenseal regularly, just mention it to your pharmacist so they can keep an eye on things.
