Dextroamphetamine (patch) falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Fever Bark interacts with.
In vitro research suggests that yohimbine, a constituent of fever bark, inhibits CYP2D6 enzyme activity. The effects of fever bark itself are unclear.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the idea with your Xelstrym (dextroamphetamine) patch and fever bark. One of the plant compounds in fever bark, yohimbine, showed in lab dish studies that it can slow down CYP2D6, an enzyme that helps clear amphetamine from your body. If that enzyme is blocked, more of the drug could linger, meaning stronger stimulant effects. Realistically that might look like a faster or pounding heartbeat, higher blood pressure, jitteriness, trouble sleeping, or feeling anxious or restless.
I want to be honest though, this is only a theory based on test-tube work, not real people, so we truly don't know how much fever bark itself would matter. Since both can raise blood pressure and heart rate on their own, it's worth checking with your pharmacist or doctor before combining them.

Reserpine, a constituent of fever bark, might increase or decrease effects of sympathomimetics. Also, yohimbine, another constituent of fever bark, has sympathomimetic effects and increases blood pressure in a dose-dependent manner. It is not known if this would occur with fever bark.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Your Xelstrym patch delivers dextroamphetamine, a stimulant that already nudges up your heart rate, blood pressure, and alertness. Fever bark is the tricky part here. It carries a mix of natural alkaloids, and they can pull in opposite directions. The yohimbine-like compounds tend to raise blood pressure and add their own stimulant push, while the reserpine-like ones can blunt some of that. So the effect on your patch could go either way, which is exactly why it's hard to predict.
The honest picture: this concern is mostly theoretical and based on weak, anecdotal evidence, not solid studies. Still, doubling up stimulant-type effects could mean a racing heart, jitteriness, high blood pressure, or trouble sleeping. Fever bark can be harsh on its own, so please check with your pharmacist or doctor before combining them.

Fever bark is the bark of the Australian tree Alstonia constricta, used in traditional medicine for fever, diarrhea, and infections.
