ANESTHESIA
Why this applies to Tetracaine
Tetracaine falls under ANESTHESIA — the group Fever Bark interacts with.
Reserpine, a constituent of fever bark, has been shown to increase the risk of cardiovascular instability in patients receiving general anesthesia. It is not known if this could occur with fever bark.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the gist: fever bark contains reserpine-like compounds, and reserpine is known to make blood pressure and heart rhythm harder to control during general anesthesia. The concern on file is mostly about those big, put-you-to-sleep anesthetics, not so much a numbing agent like tetracaine.
Tetracaine is a local anesthetic. In a topical numbing cream (like Ametop) or a spinal/eye block, very little gets into your bloodstream, so a meaningful clash with fever bark is unlikely. That said, this whole concern rests on weak, anecdotal evidence, and fever bark's alkaloids can affect blood pressure on their own. If you're scheduled for any procedure with anesthesia, just tell your anesthesiologist and pharmacist you take fever bark so they can plan safely.

Fever bark is the bark of the Australian tree Alstonia constricta, used in traditional medicine for fever, diarrhea, and infections. There is very little reliable human research to support these uses, and the bark contains potent alkaloids that may cause serious side effects. It is best avoided unless used under direct medical supervision.
