Why this applies to Erythromycin
Erythromycin falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Fever Bark interacts with.
In vitro and clinical research shows that the fever bark constituent, yohimbine, is metabolized by CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to understand about this pairing. Erythromycin is one of those antibiotics that slows down a liver enzyme called CYP3A4. Fever bark contains an alkaloid called yohimbine, and your body relies on that same CYP3A4 enzyme to clear yohimbine out. So when erythromycin puts the brakes on that enzyme, yohimbine can build up higher than it normally would.
Why does that matter? Yohimbine can raise blood pressure, speed up your heart, and trigger anxiety, jitteriness, or trouble sleeping. More of it hanging around means a bigger chance of those effects. I'll be honest with you though, this is a theoretical concern based on how the chemicals behave, not real patient reports. Still, fever bark is risky on its own, so please check with your pharmacist or doctor before combining the two.

Fever bark is the bark of the Australian tree Alstonia constricta, used in traditional medicine for fever, diarrhea, and infections. There is very little reliable human research to support these uses, and the bark contains potent alkaloids that may cause serious side effects. It is best avoided unless used under direct medical supervision.

What Is Erythromycin Used For?

Erythromycin treats bacterial infections caused by susceptible organisms. Oral and intravenous forms are used for upper and lower respiratory tract infections, skin and soft-tissue infections, pertussis (whooping cough), Legionnaires' disease, sexually transmitted infections (gonorrhea, syphilis, chlamydia), listeriosis, and other conditions. Erythrocin Lactobionate IV and erythromycin lactobionate injection are specifically for patients who cannot take oral medication or who need rapid, high blood levels due to severe infection — IV therapy should be switched to oral as soon as appropriate.
Erythromycin topical solution 2% and Ery 2% Pads (swabs) are indicated only for the topical treatment of acne vulgaris (acne) and are applied directly to the skin — they are not used for internal infections.
