Bupivacaine Hci falls under ANESTHESIA — the group Fever Bark interacts with.

Theoretically, fever bark might increase the risk of cardiovascular instability with general anesthesia.
Reserpine, a constituent of fever bark, has been shown to increase the risk of cardiovascular instability in patients receiving general anesthesia. It is not known if this could occur with fever bark.

Here's the concern with bupivacaine, which is a local anesthetic often used to numb tissue during or after surgery (Xaracoll is one form). Fever bark contains reserpine-like alkaloids, and reserpine has been linked to unstable blood pressure and heart rate in people undergoing anesthesia. Bupivacaine itself can also affect the heart and blood vessels, especially if it reaches the bloodstream, so stacking these effects together could in theory make your circulation harder to control during a procedure.

To be honest, this is based on weak, anecdotal evidence, and we don't actually know that fever bark behaves the same as pure reserpine. Bupivacaine isn't a prodrug, so this isn't about activation. Still, before any surgery or numbing procedure, please tell your pharmacist or surgical team you've used fever bark.

Fever bark is the bark of the Australian tree Alstonia constricta, used in traditional medicine for fever, diarrhea, and infections.
