Lurbinectedin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldenseal interacts with.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the thing to know about pairing goldenseal with Zepzelca (lurbinectedin). Your body relies on an enzyme called CYP3A4 to break down and clear this chemo drug. Goldenseal (mainly its berberine and hydrastine) tends to slow that enzyme down. When the cleanup crew works slower, more of the drug can linger in your blood than intended.

Lurbinectedin is an active chemo agent, not a prodrug, so higher levels generally mean stronger effects, including worse side effects like low blood counts, infections, nausea, fatigue, and liver strain. This is a real concern with a potent cancer medicine, even though the goldenseal evidence comes from small studies. Please don't start or stop anything on your own. Loop in your oncology team or pharmacist first, since they manage these doses carefully.

What Is Lurbinectedin Injection Used For?

Brand name: Zepzelca

Zepzelca (lurbinectedin) injection is approved for adults with small cell lung cancer (SCLC) in two settings. First, Zepzelca is used in combination with atezolizumab (or atezolizumab and hyaluronidase-tqjs) as maintenance treatment for extensive-stage small cell lung cancer (ES-SCLC) in patients whose disease did not progress after initial treatment with those same immunotherapy drugs plus carboplatin and etoposide.

Second, Zepzelca is approved as a single agent for adults with metastatic small cell lung cancer whose cancer has worsened during or after platinum-based chemotherapy. This second use was granted accelerated approval based on tumor response rates, and continued approval depends on results from confirmatory clinical trials.
