Theoretically, goldenseal might increase the risk of hypoglycemia when used with antidiabetes drugs.
Goldenseal contains berberine. Clinical research shows that berberine can lower blood glucose levels. However, this effect has not been reported with goldenseal.

Here's the short version: goldenseal contains a compound called berberine, and in studies berberine on its own can nudge blood sugar lower. Metformin already works to bring your blood sugar down, so stacking the two could, in theory, push your numbers lower than expected. That might show up as shakiness, sweating, a racing heart, dizziness, or feeling foggy and irritable.

I want to be honest about the evidence though. This is mostly theoretical, and this blood-sugar effect has been seen with berberine itself, not specifically with goldenseal. So I'm not raising any alarms. Still, if you take both, check your glucose a bit more often at first and let your pharmacist or doctor know so they can keep an eye on things.

Theoretically, goldenseal might reduce blood levels of metformin.
In vitro research shows that goldenseal extract decreases the bioavailability of metformin, likely by interfering with transport, intestinal permeability, or other processes involved in metformin absorption. It is unclear which, if any, of metformin's transporters are inhibited by goldenseal. Goldenseal does not appear to alter the clearance or half-life of metformin.

Here's the gist for goldenseal and your metformin. In lab studies, goldenseal seems to make metformin a little harder for your gut to absorb, so less of the drug may actually get into your bloodstream. Metformin isn't a prodrug, so lower levels simply mean a weaker effect. The practical worry is that your blood sugar could creep higher than usual, which can show up as more thirst, frequent urination, or feeling tired.

The important caveat: this is based only on test-tube research, not real patients, so we honestly don't know how much it matters in everyday life. I wouldn't panic. If you'd like to use goldenseal, just loop in your pharmacist or doctor first and keep an eye on your glucose readings.
