This combination pairs goldenseal with a medicine that already contains codeine, which is an opioid that can make you feel drowsy, foggy, or slowed down. The concern here is simple overlap: goldenseal's berberine has shown mild sedative effects in animal studies, so in theory stacking it on top of codeine could add to that sleepy feeling. You might notice more tiredness, dizziness, or trouble concentrating.

Honestly, this is a theoretical worry. The added sedation hasn't actually been seen in people, so I wouldn't lose sleep over it. Still, since codeine alone can slow your breathing and thinking, it's worth a quick chat with your pharmacist or doctor before combining the two, especially if you're already feeling groggy on this medication.

Here's the wrinkle with this combo. The codeine part of your medicine is what we call a prodrug, meaning it doesn't do much until your body converts it into morphine, and that conversion depends on an enzyme called CYP2D6. Goldenseal has been shown to slow that same enzyme down. So instead of building up codeine's effect, blocking CYP2D6 actually means less of your codeine gets turned into active morphine.

In plain terms, you may get weaker pain relief than expected. The aspirin side isn't really affected here. This is rated moderate and likely, but the human evidence is fairly limited. Before pairing goldenseal supplements with this medicine, please check with your pharmacist or doctor.

Your combo pill pairs aspirin (a blood thinner that keeps platelets from clumping) with codeine for pain. Goldenseal contains berberine, and in test-tube and animal studies berberine also seems to make platelets less sticky. Put those two together and, in theory, you could nudge up your bleeding risk a little. Realistically that might look like easier bruising, nosebleeds, or bleeding gums that takes a bit longer to stop.

Here's the honest part: this platelet effect from goldenseal has not actually been shown in people, so the concern is theoretical rather than proven. Still, since aspirin already thins your blood, it's worth mentioning to your pharmacist or doctor before you start a concentrated goldenseal supplement, especially if you bleed or bruise easily.
