Goldenseal contains berberine. Animal research shows that berberine can have sedative effects. However, this effect has not been reported in humans.

This combination pill pairs aspirin with two ingredients that already slow down your nervous system: codeine (an opioid pain reliever) and phenobarbital (a barbiturate sedative). On paper, goldenseal may add to that calming, drowsy effect. The reason is berberine, a compound in goldenseal that showed mild sedative effects in animal studies. Importantly, this hasn't actually been reported in people, so the concern is theoretical rather than proven.

If there were any added effect, you might feel extra sleepy, lightheaded, or foggy, and in larger amounts slowed breathing is the worry with codeine and phenobarbital together. I wouldn't panic, but I'd mention goldenseal to your pharmacist or doctor before combining them, especially since you'd be doubling up on sedating ingredients.

Clinical and in vitro research shows that goldenseal can significantly inhibit CYP2D6 enzymes, potentially increasing levels of drugs metabolized by CYP2D6.

This combination product has three ingredients, but the one that matters most here is the codeine. Codeine is a prodrug, meaning your body has to convert it into morphine using an enzyme called CYP2D6 before it can actually relieve pain. Goldenseal contains berberine, which can block that very enzyme.

So in this case, goldenseal could actually make the codeine less effective, because less of it gets turned into active morphine. You might notice the pain relief just doesn't work as well as expected. The aspirin and phenobarbital parts aren't the concern with this particular interaction. The evidence here is moderate, not rock solid, but it's worth a quick chat with your pharmacist or doctor before pairing them.

This combination pill pairs aspirin (a blood thinner of sorts that blocks platelets), with codeine for pain and phenobarbital, a sedative. The piece that matters here is the aspirin. Aspirin already makes your platelets less sticky, which is why it can cause easy bruising, nosebleeds, or stomach upset.

Goldenseal contains berberine, and in lab dishes and animal studies berberine also seems to slow platelet clumping. Stack that on top of aspirin and, in theory, you could see a bit more bleeding or bruising. I want to be honest though: this effect has not actually been shown in people, so the concern is more on paper than proven. It's worth a quick chat with your pharmacist or doctor before adding goldenseal, especially if you bruise easily or notice unusual bleeding.
