Saxagliptin, metformin XR falls under ANTIDIABETES DRUGS — the group Berberine interacts with.

Theoretically, berberine may increase the risk of hypoglycemia when taken with antidiabetes drugs.
Clinical research shows that berberine may lower blood glucose levels. Theoretically, berberine might have additive effects with antidiabetes drugs and increase the risk of hypoglycemia.

Kombiglyze pairs two blood sugar lowering medicines: saxagliptin and extended-release metformin. Berberine has its own blood sugar lowering activity, so stacking it on top of your prescription could push your glucose down further than expected. The concern here is additive lowering, meaning the combined effect may be stronger than either one alone.

If your numbers drop too low, you might feel shaky, sweaty, dizzy, irritable, or unusually tired. Keep in mind this is a theoretical caution based on how these work, not a strong clinical warning, so it does not mean trouble is guaranteed. Still, it is worth a quick chat with your pharmacist or doctor before adding berberine, and watch your readings more closely if you do.

Theoretically, berberine might increase the therapeutic and adverse effects of metformin.
In vitro and animal studies show that berberine can increase the systemic exposure and half-life of metformin, potentially increasing metformin's effects and side effects. This interaction seems to be most apparent when berberine is administered 2 hours prior to metformin. Taking berberine and metformin at the same time does not appear to increase systemic exposure to metformin.

Here's the thing to keep in mind with Kombiglyze (which combines saxagliptin and the extended-release form of metformin) and berberine. The research we have, mostly from lab and animal studies, suggests berberine may slow down how quickly your body clears the metformin portion. That can let more metformin linger in your system, which could nudge both its benefits and its side effects upward.

In practice that might mean stronger blood sugar lowering, but also more of metformin's usual complaints like nausea, diarrhea, or stomach cramps. Interestingly, the effect seemed strongest when berberine was taken about 2 hours before the medicine, not at the same time. Since berberine itself lowers blood sugar too, please loop in your pharmacist or doctor before combining them so they can watch your levels.
