Theoretically, berberine might increase serum levels of drugs metabolized by CYP2D6.
In vitro research and preliminary clinical evidence show that berberine can inhibit CYP2D6. Theoretically, use of berberine with drugs metabolized by CYP2D6 might increase drug levels and increase the risk of adverse effects.

Here's the wrinkle with this combo product. The piece that matters is the codeine, and codeine is a prodrug. Your body has to convert it using an enzyme called CYP2D6 into morphine, which is what actually relieves pain. Berberine appears to slow that same enzyme down a bit. So in theory, less codeine gets turned into morphine, which could mean weaker pain relief than you'd expect.

I'll be honest, this is mostly theoretical and based on lower quality research, so I wouldn't lose sleep over it. The aspirin, aluminum hydroxide, and magnesium hydroxide parts aren't really affected. If you take berberine regularly and notice your pain isn't well controlled, just loop in your pharmacist or doctor before changing anything.

In vitro and in vivo research suggest that berberine can inhibit platelet aggregation. Theoretically, berberine might have additive effects when used with anticoagulant and antiplatelet drugs and increase the risk of bleeding.

The piece of this combo product that matters here is the aspirin. Aspirin already works by making your platelets less sticky, which is how it helps prevent clots, but it's also why it can cause bruising, nosebleeds, or stomach bleeding. Lab and animal studies hint that berberine may also slow platelet clumping, so stacking the two could, in theory, add up and raise your bleeding risk a little.

To be fair, this is mostly test-tube and animal evidence, not strong human data, so it's a possibility rather than a sure thing. Still, watch for easy bruising, bleeding gums, or dark stools. Don't start or stop anything on your own. Just run it by your pharmacist or doctor first.
