Why this applies to Erdafitinib

Erdafitinib falls under Cytochrome P450 2C9 (Cyp2C9) Substrates — the group Puravive interacts with.

Puravive contains quercetin, which has been shown in studies to mildly inhibit the CYP2C9 metabolizing enzyme. This inhibition could potentially increase the risk of side effects from drugs. A small clinical trial with healthy volunteers found that taking 500 mg of quercetin twice daily for 10 days before using diclofenac, a drug metabolized by CYP2C9, increased diclofenac plasma levels by 75% and extended its half-life by 32.5% (source: https://pubmed.ncbi.nlm.nih.gov/29168292/). Combining quercetin with drugs metabolized by CYP2C9 could increase the risk of side effects, so caution is advised.

Here's the gist for Balversa (erdafitinib): your liver uses an enzyme called CYP2C9 to help clear it from your body. Puravive contains quercetin, which can mildly slow that enzyme down. When the cleanup slows, more erdafitinib can hang around in your blood, which may bump up its side effects. With this drug that could mean more mouth sores, dry eyes or vision changes, high phosphate levels, nail or skin problems, and diarrhea.

This is a cancer medicine with careful dosing, so even small shifts matter. The evidence here is moderate and based mostly on a quercetin study with a different drug, so it's a reasonable caution, not a certainty. Please don't start or stop anything on your own. Loop in your oncology team or pharmacist first.

Erdafitinib also falls under Cytochrome P450 3A4 (Cyp3A4) Substrates.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the gist: Puravive contains Panax ginseng, and your bladder cancer medication erdafitinib (Balversa) is processed by a liver enzyme called CYP3A4. Ginseng has shown mixed effects on this enzyme in studies, sometimes slowing it down and sometimes speeding it up, so we honestly can't predict which way it would push erdafitinib levels.

If ginseng slowed the enzyme, more drug could build up, which might worsen erdafitinib side effects like dry mouth and eyes, mouth sores, nail changes, or vision problems. If it sped the enzyme up, the drug could be cleared faster and work less well against the cancer. The real-world evidence here is weak and theoretical, but because this is a cancer treatment where dosing really matters, please run Puravive by your oncologist or pharmacist before starting it.

About Erdafitinib

What Is Erdafitinib Used For?

Brand name: Balversa

Balversa (erdafitinib) is used to treat adults with locally advanced or metastatic urothelial carcinoma (UC) — cancer of the bladder or urinary tract — that has spread and has not responded to, or has gotten worse after, at least one prior round of systemic treatment. Patients must have a specific type of FGFR3 genetic alteration in their tumor, confirmed by an FDA-approved test before starting.

Balversa is not recommended for patients who are eligible for — but have not yet received — a PD-1 or PD-L1 inhibitor (a type of immunotherapy). Those therapies should be tried first when appropriate.
