Puravive, a dietary supplement, contains several ingredients that may interact with anticoagulant drugs due to their potential mild anticoagulant effects. These ingredients include Kudzu, Panax ginseng, Phellodendron, and Propolis. Ginsenosides in Panax ginseng may decrease platelet aggregation in lab studies, but human studies have not confirmed this effect. Animal studies suggest low oral bioavailability of certain compounds in ginseng, which could explain differences between lab and human research. Propolis water extract and caffeic acid phenethyl ester, found in propolis, have also been shown to inhibit platelet aggregation in lab studies. Additionally, in an animal model, propolis taken with warfarin reduced the effectiveness of warfarin, suggesting a potential interaction with anticoagulant drugs. Lastly, Phellodendron contains berberine, which studies show may inhibit platelet aggregation.

The piece of your combo product that matters most here is the aspirin. Aspirin already thins the blood by quieting platelets, the cells that help you clot. Several ingredients in Puravive (ginseng, propolis, kudzu, and the berberine in phellodendron) have shown mild platelet-slowing activity in lab tests. Stack those on top of aspirin and, in theory, you could bleed or bruise a little more easily, think nosebleeds, bleeding gums, or a cut that takes longer to stop.

I want to be honest that this is theoretical and weakly supported. Most evidence is from test tubes and animals, not people, so for many folks nothing dramatic happens. Still, since codeine and phenobarbital can already make you drowsy, please run Puravive past your pharmacist or doctor before mixing it in.

Cytochrome P450 2D6 (Cyp2D6) Substrates

Aspirin, Codeine Phosphate, Phenobarbital also falls under Cytochrome P450 2D6 (Cyp2D6) Substrates.

Puravive contains Panax ginseng. Some research suggests that Panax ginseng might slow down the activity of an enzyme called CYP2D6 by about 6%. This enzyme plays a role in breaking down drugs in our system. One study (https://pubmed.ncbi.nlm.nih.gov/32557706/) found that Panax ginseng slows down the breakdown of dextromethorphan, a drug that is processed by CYP2D6, but only to a small extent. It is important to note that not all studies agree on these effects, and some suggest that Panax ginseng might not have a significant impact on CYP2D6 at all. Nevertheless, it's important to be cautious if you're taking medications that are processed by CYP2D6 and consider discussing it with your healthcare provider.

Here's the piece that matters in this combo product: the codeine. Codeine is a prodrug, meaning your body has to convert it into morphine using the CYP2D6 enzyme before it can ease your pain. The Panax ginseng inside Puravive may slow CYP2D6 down a touch (one study suggested only about 6%). If that enzyme is nudged slower, you could turn less codeine into morphine, so you might get slightly less pain relief than expected.

Honestly, this effect is small, inconsistent across studies, and rated minor. The aspirin and phenobarbital parts aren't really involved here. Still, if you notice your pain isn't as well controlled, don't change anything on your own. Just check in with your pharmacist or doctor first.
