Bazedoxifene Acetate, Conjugated Estrogens falls under Cytochrome P450 3A4 (Cyp3A4) Substrates — the group Puravive interacts with.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the situation with Duavee (conjugated estrogens plus bazedoxifene) and Puravive. Puravive contains Panax ginseng, which can nudge an enzyme called CYP3A4. That same enzyme helps your body process the bazedoxifene part of Duavee. The tricky thing is that ginseng has gone both ways in studies, sometimes slowing this enzyme and sometimes speeding it up, so we genuinely can't predict the direction here.

In theory, if ginseng sped things up, your Duavee could clear faster and work less well, meaning hot flashes might creep back. If it slowed things down, you could see more estrogen-type effects like breast tenderness, bloating, or spotting. Honestly, this is rated minor with weak evidence, so I wouldn't panic. Just run Puravive by your pharmacist or doctor first.

P-Glycoprotein Substrates

Bazedoxifene Acetate, Conjugated Estrogens also falls under P-Glycoprotein Substrates.

Puravive contains quercetin. Theoretically, concurrent use of quercetin and P-glycoprotein substrates might alter the effects and adverse effects of the drugs. There is preliminary evidence suggesting that quercetin inhibits the P-glycoprotein efflux pump in the gastrointestinal tract. This inhibition could potentially increase the bioavailability and serum levels of drugs transported by this pump. For example, a small study in healthy volunteers found that pretreatment with quercetin increased bioavailability and plasma levels following a single dose of cyclosporine (Neoral, Sandimmune) (source: https://pubmed.ncbi.nlm.nih.gov/15581264/). However, other small studies have shown that quercetin might decrease the absorption of talinolol, another substrate of the gastrointestinal P-glycoprotein efflux pump. Interestingly, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase) in another small study (source: https://pubmed.ncbi.nlm.nih.gov/16945047/). The reason for these discrepancies is not entirely clear. Until more is known, it is advisable to use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the gist with Duavee (estrogen plus bazedoxifene) and Puravive. Puravive contains quercetin, and in lab settings quercetin can block a gut transporter called P-glycoprotein, which helps move certain drugs in and out. Bazedoxifene is one of the medicines handled by this pump. In theory, slowing the pump could let a bit more of your medication get absorbed, which might mean slightly stronger effects or more side effects like breast tenderness, leg cramps, nausea, or fluid retention.

That said, this is theoretical and ranked as minor. The human studies on quercetin are small and even point in different directions, so we don't really know it would matter here. Before adding Puravive, run it by your pharmacist or doctor, since hormone therapy deserves a careful eye anyway.

What Is Estrogen and Bazedoxifene Used For?

Brand name: Duavee

  • Estrogen and bazedoxifene tablets are used to treat hot flashes (sudden feelings of warmth, especially in the face, neck, and chest) in women who are experiencing menopause (stage of life when menstrual periods become less frequent and stop and women may experience other symptoms and body changes). Estrogen and bazedoxifene tablets are also used to prevent osteoporosis (condition in which the bones become thin and weak and break easily) in women who have undergone menopause. Estrogen is in a class of medications called hormones and bazedoxifene is in a class of medications called estrogen agonist–antagonists. Estrogen works by replacing estrogen that is normally produced by the body. Bazedoxifene is used to block the action of estrogen on the lining of the uterus, decreasing the risk of overgrowth that may lead to cancer.
