Glecaprevir, Pibrentasvir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the concern with pairing Danshen and Mavyret (glecaprevir and pibrentasvir). Mavyret is partly handled by a liver and gut enzyme called CYP3A4. In early human research, taking Danshen for about 10 to 14 days seemed to speed that enzyme up, which could clear Mavyret out of your body faster. Lower levels matter here because Mavyret is fighting a virus, and you need steady, full drug levels to clear hepatitis C. Less drug could mean the treatment doesn't work as well.

Interestingly, a single dose of Danshen did the opposite and raised levels. The evidence is still limited and a bit mixed, so I wouldn't panic. But since finishing your hepatitis C course successfully really counts, please talk with your pharmacist or doctor before adding Danshen.

Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist for Mavyret (glecaprevir/pibrentasvir). Both of these antiviral ingredients ride through your body partly on a transporter called P-glycoprotein, which acts like a pump that helps move drugs around and out. Danshen appears to slow that pump down a bit. In a small study, danshen raised levels of another P-glycoprotein drug by roughly 30 to 40 percent, so it's reasonable to think your Mavyret levels could edge up too.

Higher levels could mean more of the usual side effects, like headache, tiredness, or nausea. The evidence here is limited and based on a different drug, so this is more of a caution than a certainty. Since you're treating hepatitis C and want this medicine working just right, please check with your pharmacist or doctor before adding danshen rather than starting it on your own.

What Is Glecaprevir and Pibrentasvir Used For?

Brand name: Mavyret

Mavyret treats acute or chronic hepatitis C virus (HCV) infection caused by genotypes 1, 2, 3, 4, 5, or 6 in adults and children 3 years and older who have no cirrhosis or have compensated cirrhosis (mild liver scarring that is still functioning).

Mavyret is also approved for adults and children 3 years and older with genotype 1 HCV who were previously treated with either an NS5A inhibitor or an NS3/4A protease inhibitor (two earlier classes of hepatitis C drugs) — but not both at the same time.
