Theoretically, taking danshen with antihypertensive drugs might increase the risk of hypotension.
Animal research suggests that danshen can produce dose-dependent hypotensive effects. Furthermore, concomitant use with captopril appears to potentiate this effect.

Your Diovan HCT combines two blood pressure lowerers: valsartan, which relaxes blood vessels, and hydrochlorothiazide, a water pill that helps your body shed extra fluid and salt. Both work to bring your pressure down. Here's the thing with Danshen: in animal studies it also appears to lower blood pressure on its own, and it seemed to add to the effect when paired with a blood pressure medicine. So stacking it on top of your Diovan HCT could, in theory, push your pressure a little too low.

If that happens, you might feel dizzy, lightheaded, tired, or woozy when you stand up. I'll be honest, this concern comes from lab and animal data, not solid human studies, so it's more a heads-up than a red alert. Still, please check with your pharmacist or doctor before adding Danshen.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Hydrochlorothiazide, Valsartan also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP2C9.
In vitro research shows that various constituents of danshen inhibit the activity of CYP2C9. So far, this interaction has not been reported in humans.

Here's the situation with Diovan HCT (valsartan plus hydrochlorothiazide) and Danshen. Lab studies suggest Danshen can block a liver enzyme called CYP2C9. Valsartan is broken down partly by this enzyme, so in theory Danshen could let a little more valsartan build up in your system. That might nudge its blood-pressure-lowering effect a bit stronger, possibly bringing on dizziness, lightheadedness, or feeling faint when you stand up.

I want to be honest though: this is only seen in test-tube research and has never actually been reported in people, so it's a theoretical concern, not a proven problem. Danshen on its own can also affect blood clotting, which is worth mentioning to your team. Before adding it, just check with your pharmacist or doctor.
