Why this applies to Omeprazole

Omeprazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Kratom interacts with.

Kratom might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that kratom extract inhibits CYP3A4 enzyme activity. A pharmacokinetic study in adults shows that taking kratom tea 2 grams modestly increases the plasma concentration time-curve (AUC) and maximum concentration (Cmax) of CYP3A4 probe midazolam by 40-50% and its CYP3A4-mediated metabolites 18-27%. However, a lack of change in the half-life of midazolam and a simulation of the kratom-midazolam interaction suggest that kratom inhibits CYP3A4 enzymes in the small intestine but not the liver. Additionally, there is one case report of a 27-year-old male who died from neuroleptic malignant-like symptoms after concomitant use of kratom and quetiapine, a CYP3A4 substrate. Although it did not appear that the patient had consumed large quantities of quetiapine, postmortem plasma levels were in the lethal range, indicating possible inhibition of CYP3A4 by kratom. In another case report, a 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It is hypothesized that CYP3A4 inhibition by kratom reduced metabolism of desvenlafaxine, trazodone, and ziprasidone and consequently increased systemic exposure to serotonin.

Here's the gist for omeprazole: your body clears it using liver enzymes, mainly one called CYP2C19, with some help from CYP3A4. Kratom may slow down CYP3A4, and lab studies suggest that could let a bit more omeprazole linger in your system. In theory, that means slightly stronger or longer-lasting effects, which for omeprazole usually just means a touch more stomach acid suppression rather than dramatic side effects.

Honestly, this one is more theoretical than proven. The signal comes mostly from test-tube work, and omeprazole isn't a prodrug, so I wouldn't expect a major problem. My bigger concern is kratom itself, which carries real risks. Please loop in your pharmacist or doctor before mixing them.

Here's the gist for omeprazole (Prilosec): your liver uses an enzyme called CYP2C19 to clear this medicine out. Lab dish studies suggest kratom can weakly slow that same enzyme down. If that happened in real life, omeprazole might hang around a little longer and build up slightly, which in theory could mean a touch more of its usual effects or side effects, like headache, nausea, or stomach upset.

I want to be honest with you though: this is purely theoretical, based on test-tube work, and the inhibition was described as weak. We don't have real-world cases showing trouble, and omeprazole is a pretty forgiving drug. The bigger concern is kratom itself, which carries real risks. Please loop in your pharmacist or doctor before combining the two.

What Is Omeprazole Used For?

Brand names: Prilosec, Zegerid, Losec

Omeprazole is used to treat a range of conditions where stomach acid causes problems. These include heartburn and acid reflux (GERD), erosive esophagitis (damage to the esophagus from acid), and stomach or duodenal ulcers. It can also be used alongside antibiotics to help get rid of an H. pylori infection — a bacteria that causes ulcers.

In more serious cases, omeprazole is used to manage conditions where the stomach produces far too much acid, such as pathologic hypersecretory conditions (rare medical disorders that cause extreme acid overproduction). It is approved for adults and children one year of age and older for GERD and erosive esophagitis.
