CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Omeprazole falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Quercetin interacts with.
A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: your body uses a liver enzyme called CYP2C9 as one of the pathways that clears omeprazole. Quercetin, in concentrated supplement form, may slow that enzyme down a bit. If omeprazole gets cleared more slowly, slightly more of it could hang around in your blood, which could nudge up its acid-blocking effect and any side effects like headache, nausea, gas, or loose stools.
Honestly, though, this is mostly theoretical. The human evidence comes from other CYP2C9 drugs, not omeprazole itself, and quercetin in everyday food amounts (apples, onions, tea) is unlikely to matter. High-dose supplements are the real question. If you're considering one, just check with your pharmacist or doctor first before changing anything.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the gist with omeprazole and quercetin. Your body clears omeprazole using liver enzymes, and CYP3A4 is one of them (though CYP2C19 actually does most of the heavy lifting for this drug). Quercetin can nudge that enzyme system, but the lab studies pull in both directions. Some suggest it blocks CYP3A4 (which could let omeprazole linger a bit), while another human study hinted it might speed the enzyme up.
Honestly, this is mostly test-tube and animal data, so the real-world effect for omeprazole is likely small. Food amounts of quercetin from fruits and veggies are nothing to worry about. If you're taking a concentrated supplement and notice more headache or stomach upset, or your heartburn creeps back, just check in with your pharmacist or doctor before changing anything.

What Is Omeprazole Used For?

Omeprazole is used to treat a range of conditions where stomach acid causes problems. These include heartburn and acid reflux (GERD), erosive esophagitis (damage to the esophagus from acid), and stomach or duodenal ulcers. It can also be used alongside antibiotics to help get rid of an H. pylori infection — a bacteria that causes ulcers.
In more serious cases, omeprazole is used to manage conditions where the stomach produces far too much acid, such as pathologic hypersecretory conditions (rare medical disorders that cause extreme acid overproduction). It is approved for adults and children one year of age and older for GERD and erosive esophagitis.
