Why this applies to Propranolol Hydrochloride

Propranolol Hydrochloride falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Kratom interacts with.

Theoretically, kratom might increase the levels and clinical effects of drugs metabolized by CYP2D6.
In vitro research suggests that kratom extract inhibits CYP2D6 enzyme activity. However, human research shows that a single low dose of kratom tea 2 grams does not inhibit CYP2D6 activity. However, this kratom dose is lower than what is normally taken for psychoactive effects. In one case report, a 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It was hypothesized that kratom inhibited CYP2D6-mediated metabolism of buspirone and consequently increased systemic exposure to serotonin. In another case, a 37-year-old male stable for 15 years on amitriptyline presented with anticholinergic symptoms including xerostomia, dry eyes, and constipation, along with mildly elevated bilirubin and liver enzymes after taking progressively higher doses of up to 14 grams of kratom daily for 12 weeks. It was hypothesized that the adverse effects of amitriptyline were precipitated by CYP2D6 inhibition by kratom.

Here's the gist for propranolol. Your body relies on a liver enzyme called CYP2D6 to help clear this medicine. Kratom may slow that enzyme down, so in theory propranolol could build up a bit more than expected and hit harder than intended. With this drug, stronger effects could mean a slower heart rate, lower blood pressure, dizziness, or unusual tiredness. In a baby being treated for a hemangioma, that's something you'd want to watch closely.

I want to be honest though: this is a theoretical concern based mostly on lab studies, and a small dose of kratom did not block the enzyme in people. Still, kratom carries serious risks of its own, so please loop in your doctor or pharmacist before combining anything.

Theoretically, kratom might increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research suggests that kratom extract inhibits the activity of CYP1A2. In one case report, a 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It was hypothesized that kratom inhibited CYP1A2-mediaed metabolism of ziprasidone which consequently increased systemic exposure to serotonin.

Here's the gist for propranolol. Your liver uses an enzyme called CYP1A2 to help clear this medicine from the body. Lab studies hint that kratom may slow that same enzyme down. If that happens, propranolol could build up a bit more than expected, which might lead to stronger beta-blocker effects like a slower heart rate, lower blood pressure, dizziness, tiredness, or feeling faint. Propranolol is not a prodrug, so more drug in the system generally means more effect, not less.

I want to be honest that this is theoretical, based mostly on test-tube work rather than real patients. Still, kratom itself carries serious risks. Please loop in your pharmacist or doctor before combining the two.

Here's the short version on mixing kratom with propranolol. Propranolol is an active beta-blocker (not a prodrug), so it doesn't need to be switched on by your body to work. The concern on file is that kratom may weakly block an enzyme called CYP2C19, which helps clear some drugs. If that enzyme slows down, propranolol could build up a little, which in theory might nudge its effects up: a slower heart rate, lower blood pressure, tiredness, or feeling lightheaded.

That said, this is a theoretical, lab-only finding, and propranolol is mostly handled by a different enzyme (CYP2D6), so the real-world impact is likely small. Since this oral solution is given to babies for hemangiomas, please loop in your child's doctor or pharmacist before combining anything with it.

This intravenous (IV) form of propranolol is used in hospital settings to treat serious, life-threatening heart rhythm problems. It works on several types of abnormal rhythms, including those that happen during anesthesia or are caused by drug toxicity.

Specific uses include fast heart rhythms coming from the upper chambers of the heart (supraventricular tachycardia), dangerous rhythms caused by too much digitalis in the body, and certain fast rhythms triggered by excess stress hormones (catecholamines) during surgery.
