Phenylephrine falls under SEROTONERGIC DRUGS — the group Kratom interacts with.

Theoretically, taking kratom in combination with serotonergic drugs may increase levels of serotonin, increasing the risk of serotonin syndrome.
A 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It was hypothesized that kratom inhibited the cytochrome P450 3A4-, 2C9-, 2D6-, and 1A2-mediated metabolism of several of these serotonergic medications, consequently increasing systemic exposure to serotonin.

Here's the thing to know if you're reaching for a Sudafed PE or another phenylephrine decongestant while using kratom. The interaction we have on file is about serotonin syndrome, where too much serotonin activity builds up in the body. Honestly, this concern fits much better with antidepressants and similar drugs than it does with phenylephrine, which mainly works by tightening blood vessels to clear nasal congestion.

That said, both kratom and phenylephrine can rev things up a bit, so the more realistic worry with this pair is a bump in blood pressure, a faster or pounding heartbeat, jitteriness, or trouble sleeping. This is theoretical and based only on weak, anecdotal evidence, so I don't want to alarm you. Still, please run it by your pharmacist or doctor first, especially if you have high blood pressure or heart concerns.

What Is Phenylephrine Used For?

Brand names: Gilchew, AH Chew D, Gilchew IR, Sudafed PE, Biorphen

Phenylephrine is used in several different products, and each one has its own use.

Injection (IV): raises blood pressure in adults with clinically important hypotension (low blood pressure) caused mainly by widened blood vessels. Some injections are labeled for anesthesia only. Others are labeled for anesthesia and septic shock.

Eye drops: Phenylephrine Hydrochloride Ophthalmic Solution is used to dilate the pupil.

Nasal products: temporarily relieve nasal and sinus congestion from the common cold, hay fever or other upper respiratory allergies.

Rectal suppositories: relieve itching, burning and discomfort from hemorrhoids and temporarily shrink hemorrhoidal tissue.
