Nilutamide falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Kratom interacts with.

Theoretically, kratom might increase the levels of drugs metabolized by CYP2C19.
In vitro research shows that kratom extract weakly inhibits CYP2C19 enzyme activity.

Here's the gist with kratom and your prostate cancer pill, nilutamide. Your body uses a liver enzyme called CYP2C19 to help clear nilutamide out. Kratom may slow that same enzyme down a little, so in theory nilutamide could build up a bit and hang around longer than usual. Nilutamide is an active drug (not a prodrug), so more of it means a stronger chance of its usual side effects, like nausea, hot flashes, trouble seeing in the dark or adjusting to bright light, dizziness, or feeling more tired.

That said, this concern is theoretical and comes from lab dish testing only, where kratom blocked the enzyme only weakly. It hasn't been proven in real people. Still, kratom carries its own serious risks, so please talk with your pharmacist or oncologist before mixing the two.

What Is Nilutamide Used For?

Brand names: Nilandron, Anandron

Nilutamide tablets are used together with surgical castration (removal of the testicles) to treat metastatic prostate cancer — prostate cancer that has spread to other parts of the body (Stage D2). It is not used alone; it works as part of this combined treatment approach.

For the best results, treatment with nilutamide should begin on the same day as surgical castration, or the day after.
