Why this applies to Formoterol

Formoterol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Kratom interacts with.

Theoretically, kratom might increase the levels and clinical effects of drugs metabolized by CYP2D6.
In vitro research suggests that kratom extract inhibits CYP2D6 enzyme activity. However, human research shows that a single low dose of kratom tea 2 grams does not inhibit CYP2D6 activity. However, this kratom dose is lower than what is normally taken for psychoactive effects. In one case report, a 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It was hypothesized that kratom inhibited CYP2D6-mediated metabolism of buspirone and consequently increased systemic exposure to serotonin. In another case, a 37-year-old male stable for 15 years on amitriptyline presented with anticholinergic symptoms including xerostomia, dry eyes, and constipation, along with mildly elevated bilirubin and liver enzymes after taking progressively higher doses of up to 14 grams of kratom daily for 12 weeks. It was hypothesized that the adverse effects of amitriptyline were precipitated by CYP2D6 inhibition by kratom.

Here's the gist with your Foradil (formoterol). Your body partly uses a liver enzyme called CYP2D6 to clear this inhaled medicine. In lab studies, kratom appears to slow that same enzyme down. So in theory, if kratom blocks CYP2D6, more formoterol could hang around in your system, which might nudge up the usual side effects you'd see with a long-acting beta agonist: a racing or pounding heart, jittery or shaky hands, headache, or a feeling of being wired.

I want to be honest that this is theoretical, based mostly on test-tube work, and one low dose of kratom tea didn't actually block the enzyme in people. Still, the doses folks take for effect are much higher. Since formoterol already affects the heart, please check with your pharmacist or doctor before mixing the two.

Here's the honest picture with formoterol and kratom. The concern on file is that kratom may mildly slow down a liver enzyme called CYP2C19, which could let drugs that depend on it build up a little. But this is only a theoretical idea from lab dish studies, where kratom blocked the enzyme only weakly. There's no real-world evidence of a problem here.

On top of that, formoterol is an inhaled medicine that's not heavily processed by CYP2C19, so this pathway likely matters very little for it. If anything pushed its levels up, you might notice more jitteriness, a fast or pounding heartbeat, or shakiness. The bigger issue is kratom's own risks. Please talk with your pharmacist or doctor before combining them.

What Is Formoterol Oral Inhalation Used For?

Brand names: Foradil Aerolizer, Atimos Modulite, Oxis Turbohaler, Perforomist, Foradil Certihaler

Formoterol fumarate inhalation solution is used for the long-term maintenance treatment of airway narrowing (bronchoconstriction) in adults with COPD, including chronic bronchitis and emphysema. It is taken twice daily — morning and evening — as a regular, ongoing treatment.

Important limits: This medication is not for relieving sudden COPD flare-ups or breathlessness in the moment, and it is not approved to treat asthma. A separate short-acting inhaler should always be on hand for sudden symptoms.
