Why this applies to Formoterol

Formoterol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group White Pepper interacts with.

In vitro evidence suggests that some constituents of white pepper inhibit cytochrome P450 2D6 (CYP2D6). Theoretically, concomitant use may affect drugs metabolized by CYP2D6.
Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.

Here's the gist with your formoterol inhaler and white pepper. Formoterol is partly broken down by an enzyme called CYP2D6. Lab studies hint that piperine, the spicy compound in white pepper, might slow that enzyme down a little. In theory, that could let a bit more formoterol linger in your system, which might nudge up the usual side effects like a fast or pounding heartbeat, jitteriness, shakiness, or a headache.

That said, this is theoretical and based only on test-tube work, not real people. The sprinkle of white pepper you'd use in cooking is very unlikely to matter. Concentrated piperine supplements are the bigger question mark. If you're considering one, just run it by your pharmacist or doctor first.

What Is Formoterol Oral Inhalation Used For?

Brand names: Foradil Aerolizer, Atimos Modulite, Oxis Turbohaler, Perforomist, Foradil Certihaler

Formoterol fumarate inhalation solution is used for the long-term maintenance treatment of airway narrowing (bronchoconstriction) in adults with COPD, including chronic bronchitis and emphysema. It is taken twice daily — morning and evening — as a regular, ongoing treatment.

Important limits: This medication is not for relieving sudden COPD flare-ups or breathlessness in the moment, and it is not approved to treat asthma. A separate short-acting inhaler should always be on hand for sudden symptoms.
