Hydrocortisone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group White Pepper interacts with.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits cytochrome P450 3A4 (CYP3A4). Theoretically, concomitant use with white pepper might increase the effects and side effects of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the gist: your body uses a liver enzyme called CYP3A4 to break down hydrocortisone. Piperine, the spicy compound in white pepper, has shown in lab studies that it can slow that same enzyme down. In theory, that could let a bit more hydrocortisone hang around in your body, nudging up both its effects and its possible side effects, things like fluid retention, raised blood sugar, mood changes, or trouble sleeping.

The important context: this is theoretical, based on test-tube work, not real patient cases. The sprinkle of pepper on your dinner is very unlikely to cause any trouble. Concentrated piperine or black pepper supplements are where I'd pause. If you're taking those regularly, just run it by your pharmacist or doctor first.

P-GLYCOPROTEIN SUBSTRATES

Hydrocortisone also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, white pepper might increase levels of p-glycoprotein substrates. The piperine constituent of white pepper seems to inhibit p-glycoprotein in vitro.
Some drugs metabolized by p-glycoprotein include some chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), protease inhibitors (amprenavir, indinavir, nelfinavir, saquinavir), H2 antagonists (cimetidine, ranitidine), some calcium channel blockers (diltiazem, verapamil), digoxin, corticosteroids, erythromycin, cisapride (Propulsid), fexofenadine (Allegra), cyclosporine, loperamide (Imodium), quinidine, and others.

Here's the gist: hydrocortisone is one of many medicines that ride along on a transporter called p-glycoprotein, which acts like a tiny pump moving the drug in and out of your cells and gut. The piperine in white pepper has shown, in lab dishes only, that it can slow that pump down. If that happened in your body, a bit more hydrocortisone could stick around, possibly nudging up steroid-type effects like fluid retention, higher blood sugar, mood changes, or stomach upset.

The honest part: this is theoretical, based on test-tube data, not real patient cases. Everyday cooking amounts of white pepper are very unlikely to matter. If you take concentrated piperine supplements, just run it by your pharmacist or doctor first.

Brand names: Cortef, Hydrocortone, Carmol HC cream, Hydrocortisone Iodoquinol Cream, Vytone Cream, Massengil Medicated Towelettes, Dioderm, Efcortelan Cream, Mildison, Alphaderm, Calmurid HC, Efcortelan Ointment, Texacort, Alkindi Sprinkle

Hydrocortisone is used to calm skin inflammation and relieve itching caused by conditions that respond to corticosteroid treatment. In adults, it treats a broad range of inflammatory skin conditions. In children as young as 3 months old, it is specifically approved for mild to moderate atopic dermatitis (eczema).

It is also used in rectal foam form to treat inflammation of the lower bowel, such as ulcerative proctitis.
