Tisotumab Vedotin-Tftv falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group White Pepper interacts with.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits cytochrome P450 3A4 (CYP3A4). Theoretically, concomitant use with white pepper might increase the effects and side effects of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the gist: white pepper contains piperine, and in test-tube studies piperine can mildly block a liver enzyme called CYP3A4. The small-molecule chemotherapy part of Tivdak (a cell-killing payload called MMAE) is partly cleared by CYP3A4. So in theory, slowing that enzyme could let a bit more of the drug linger, which might mean stronger side effects like nerve tingling or numbness in the hands and feet, fatigue, nausea, low blood counts, or eye irritation.

That said, this is theoretical and based only on lab data, not real patient cases. Normal cooking amounts of white pepper are very unlikely to matter. Concentrated piperine supplements are a different story, so check with your oncology team or pharmacist before using them during treatment.
