Nifedipine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group White Pepper interacts with.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits cytochrome P450 3A4 (CYP3A4). Theoretically, concomitant use with white pepper might increase the effects and side effects of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the gist for nifedipine: your body relies on an enzyme called CYP3A4 to break it down and clear it out. The piperine in white pepper has shown, in lab studies only, an ability to slow that enzyme. If it does that in real life, nifedipine could linger longer and hit a bit harder, which might mean more of its usual effects like dizziness, flushing, headache, swelling in the ankles, or blood pressure dropping lower than you'd want.

The reassuring part: this is theoretical, based on test-tube data, not human studies. Sprinkling white pepper on your food is very unlikely to cause trouble. Concentrated piperine supplements are the bigger question mark. If you're considering one, just run it by your pharmacist or doctor first.

What Is Nifedipine Used For?

Nifedipine treats high blood pressure (hypertension), chronic stable angina (chest pain brought on by physical effort), and vasospastic angina (chest pain caused by spasm of the coronary arteries, sometimes called Prinzmetal's angina). Lowering blood pressure with nifedipine helps reduce the risk of strokes and heart attacks.

The capsule form is approved for vasospastic angina and chronic stable angina only — it is not approved for high blood pressure. The extended-release tablet (including Procardia) covers all three uses: vasospastic angina, chronic stable angina, and high blood pressure.
