White Pepper and Amoxicillin, Clavulanate Potassium

Here is the interaction we found between these two, with the evidence behind it.

Minor

AMOXICILLIN (Amoxil, Trimox)

Evidence from animal research shows that piperine, a constituent of white pepper, increases the plasma levels of amoxicillin when taken concomitantly. Theoretically, piperine from white pepper might increase the effects and side effects of amoxicillin in humans. Be watchful with this combination.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: the piperine in white pepper may help your body absorb more of the amoxicillin part of Augmentin, nudging blood levels a little higher. In theory, that could mean slightly stronger effects, but also a bit more of the common side effects this antibiotic already tends to cause, like stomach upset, nausea, or loose stools (and Augmentin is known for diarrhea thanks to the clavulanate).

Now the reassuring part. This is rated a minor interaction, and it's only theoretical, based on animal studies, not solid human research. The sprinkle of white pepper on your dinner is very unlikely to matter. Concentrated piperine supplements are the bigger question mark, so check with your pharmacist or doctor before taking those alongside your antibiotic.

From our pharmacist’s overview on the Amoxicillin, Clavulanate Potassium page

“Beer and wine sit among the six major-rated interactions for amoxicillin and clavulanate potassium (Augmentin), and this list deserves real caution. Clavulanate is a well-known antibiotic cause of liver injury, so the 38 ingredients flagged for adding liver stress carry extra weight with this particular drug.”

About White Pepper

What is White Pepper?

  • Digestive upset
  • Seasoning and appetite support
  • Cough and cold remedies (traditional)
  • Minor aches and pains (traditional)
  • Antimicrobial (traditional)

White pepper comes from the same plant as black pepper and is mostly used as a kitchen spice. In normal food amounts it is generally safe for most people, but there is little solid human research supporting its use as a medicine. Talk with your pharmacist or doctor before using concentrated supplements, especially if you are pregnant, breastfeeding, or have a health condition.
