Miconazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group White Pepper interacts with.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits cytochrome P450 3A4 (CYP3A4). Theoretically, concomitant use with white pepper might increase the effects and side effects of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the good news on this pairing: most folks use miconazole as a cream, vaginal suppository, or powder (think Monistat or Zeasorb-AF) for yeast and fungal infections. When you apply it to the skin or use it vaginally, very little gets into your bloodstream, so the white pepper concern barely applies at all.

The theory is that piperine in white pepper may mildly block CYP3A4, an enzyme that helps clear miconazole. In a lab dish, blocking that enzyme could let more drug hang around. But this is theoretical, based on test-tube data, not real people, and seasoning your food with pepper isn't likely to matter. If you ever take miconazole in an oral or stronger form, just mention your supplements to your pharmacist or doctor.
