Brexpiprazole falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group White Pepper interacts with.

In vitro evidence suggests that some constituents of white pepper inhibit cytochrome P450 2D6 (CYP2D6). Theoretically, concomitant use may affect drugs metabolized by CYP2D6.
Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.

Here's the gist: your body uses a liver enzyme called CYP2D6 to help clear brexpiprazole (Rexulti). Lab studies hint that piperine, the spicy compound in white pepper, can slow that same enzyme down. Brexpiprazole is an active drug, not a prodrug, so if its breakdown slows, more of it could linger in your system. In theory that might mean stronger effects or more side effects like drowsiness, restlessness, dizziness, or weight gain.

The important part: this is only a theoretical concern based on test-tube research, not real patient cases. Sprinkling white pepper on your food is very unlikely to cause trouble. Concentrated piperine supplements are the bigger question mark. If you're considering one, just run it by your pharmacist or doctor first.

Brexpiprazole also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits cytochrome P450 3A4 (CYP3A4). Theoretically, concomitant use with white pepper might increase the effects and side effects of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the gist with Rexulti (brexpiprazole) and white pepper. Your body relies on a liver enzyme called CYP3A4 to break down brexpiprazole. A compound in pepper called piperine can mildly block that enzyme in lab studies, so in theory it could slow how fast you clear the drug. If that happens, more brexpiprazole sticks around, which could nudge up its effects and side effects like drowsiness, restlessness, dizziness, or weight changes.

The honest truth: this is a theoretical concern based on test-tube data, not real patient cases. The amount of white pepper you sprinkle on food is very unlikely to matter. Concentrated piperine supplements are where I'd pause. If you're considering one, check with your pharmacist or doctor first.

About Brexpiprazole

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
