White Pepper and Tolazamide

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIDIABETES DRUGS

Why this applies to Tolazamide

Tolazamide falls under ANTIDIABETES DRUGS — the group White Pepper interacts with.

Evidence from animal research suggests that piperine, a constituent of white pepper, reduces blood glucose levels. Theoretically, white pepper might have additive effects with antidiabetes drugs and increase the risk of hypoglycemia. Monitor blood glucose levels closely. Dose adjustments might be necessary.
Some antidiabetes drugs include glimepiride (Amaryl), glyburide (DiaBeta, Glynase PresTab, Micronase), insulin, pioglitazone (Actos), rosiglitazone (Avandia), and others.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with tolazamide and white pepper. Tolazamide already pushes your pancreas to release more insulin, which lowers blood sugar. In animal studies, piperine (the spicy compound in pepper) also seemed to nudge blood sugar down a bit. So the worry is that the two could stack up and drop your sugar a little too far, leaving you shaky, sweaty, dizzy, or confused.

Now, I want to keep this in perspective. This is theoretical and comes from lab and animal work, not solid human studies. Sprinkling white pepper on dinner is very unlikely to cause trouble. Concentrated piperine supplements are the bigger question mark. If you go that route, check your blood sugar a bit more often at first and loop in your pharmacist or doctor before changing anything.

From our pharmacist’s overview on the Tolazamide page

“Tolazamide (Tolinase) lowers blood sugar in type 2 diabetes, and I would handle its supplement list with care, because nearly everything listed can drag blood sugar lower still. Herbs and foods with glucose-lowering effects dominate the file, and stacked on a pill that already lowers glucose they raise the odds of hypoglycemia, blood s…”

About White Pepper

What is White Pepper?

  • Digestive upset
  • Seasoning and appetite support
  • Cough and cold remedies (traditional)
  • Minor aches and pains (traditional)
  • Antimicrobial (traditional)

White pepper comes from the same plant as black pepper and is mostly used as a kitchen spice. In normal food amounts it is generally safe for most people, but there is little solid human research supporting its use as a medicine. Talk with your pharmacist or doctor before using concentrated supplements, especially if you are pregnant, breastfeeding, or have a health condition.

About Tolazamide

What Is Tolazamide Used For?

Brand name: Tolinase

  • Tolazamide is used along with diet and exercise, and sometimes with other medications, to treat type 2 diabetes (condition in which the body does not use insulin normally and, therefore, cannot control the amount of sugar in the blood). Tolazamide is in a class of medications called sulfonylureas. Tolazamide lowers blood sugar by causing the pancreas to produce insulin (a natural substance that is needed to break down sugar in the body) and helping the body use insulin efficiently. This medication will only help lower blood sugar in people whose bodies produce insulin naturally. Tolazamide is not used to treat type 1 diabetes (condition in which the body does not produce insulin and, therefore, cannot control the amount of sugar in the blood) or diabetic ketoacidosis (a serious condition that may occur if high blood sugar is not treated).

  • Over time, people who have diabetes and high blood sugar can develop serious or life-threatening complications, including heart disease, stroke, kidney problems, nerve damage, and eye problems. Taking medication(s), making lifestyle changes (e.g., diet, exercise, quitting smoking), and regularly checking your blood sugar may help to manage your diabetes and improve your health. This therapy may also decrease your chances of having a heart attack, stroke, or other diabetes-related complications such as kidney failure, nerve damage (numb, cold legs or feet; decreased sexual ability in men and women), eye problems, including changes or loss of vision, or gum disease. Your doctor and other healthcare providers will talk to you about the best way to manage your diabetes.
