Here's the thinking behind this pairing. When you take hydrocortisone, one of its main jobs is often to calm down an overactive immune system, whether that's quieting inflammation in arthritis, lupus, or a strong allergic reaction. The concern with quillaia is the opposite direction: lab studies suggest some of its saponins may actually stimulate the immune system. In theory, that nudge could work against what hydrocortisone is trying to do and make it a little less effective.

I want to be honest that this is theoretical, based on test-tube research rather than real patients, so I wouldn't lose sleep over it. The small amounts of quillaia used as a food or drink emulsifier are very unlikely to matter. If you're considering concentrated quillaia supplements while on hydrocortisone, just run it by your pharmacist or doctor first.

Quillaia (soap bark tree) is a plant whose bark contains foaming compounds called saponins, and it is widely used as an emulsifier in foods and drinks. As a supplement or herbal remedy its benefits are not well proven, and large amounts can be irritating or toxic, so it should be used cautiously and only as directed.

Brand names: Cortef, Hydrocortone, Carmol HC cream, Hydrocortisone Iodoquinol Cream, Vytone Cream, Massengil Medicated Towelettes, Dioderm, Efcortelan Cream, Mildison, Alphaderm, Calmurid HC, Efcortelan Ointment, Texacort, Alkindi Sprinkle

Hydrocortisone is used to calm skin inflammation and relieve itching caused by conditions that respond to corticosteroid treatment. In adults, it treats a broad range of inflammatory skin conditions. In children as young as 3 months old, it is specifically approved for mild to moderate atopic dermatitis (eczema).

It is also used in rectal foam form to treat inflammation of the lower bowel, such as ulcerative proctitis.
