Berotralstat Hydrochloride falls under P-GLYCOPROTEIN SUBSTRATES — the group White Pepper interacts with.

Theoretically, white pepper might increase levels of p-glycoprotein substrates. The piperine constituent of white pepper seems to inhibit p-glycoprotein in vitro.
Some drugs metabolized by p-glycoprotein include some chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), protease inhibitors (amprenavir, indinavir, nelfinavir, saquinavir), H2 antagonists (cimetidine, ranitidine), some calcium channel blockers (diltiazem, verapamil), digoxin, corticosteroids, erythromycin, cisapride (Propulsid), fexofenadine (Allegra), cyclosporine, loperamide (Imodium), quinidine, and others.

Here's the gist for Orladeyo (berotralstat), which folks take daily to prevent hereditary angioedema attacks. Your body uses a transporter pump called P-glycoprotein to help move this drug along, and lab studies suggest piperine (the spicy compound in white pepper) may slow that pump down. In theory, that could let a bit more berotralstat build up in your system, which might mean more of its usual side effects, like stomach pain, nausea, diarrhea, or headache.

I want to keep this in perspective. This is a theoretical concern based on test-tube data, not real patient reports, and the sprinkle of pepper on your dinner is very unlikely to matter. Concentrated piperine supplements are a different story. Before starting any pepper-based supplement, just run it by your pharmacist or doctor.
