Hydrocortisone falls under IMMUNOSUPPRESSANTS — the group Picrorhiza interacts with.

Picrorhiza seems to have immunostimulating activity. Theoretically, picrorhiza may interfere with immunosuppressant therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), and other corticosteroids (glucocorticoids).

Here's the thing to know about pairing Picrorhiza with hydrocortisone. This herb appears to give the immune system a little nudge to be more active. Hydrocortisone works in the opposite direction, calming the immune system down to ease inflammation, allergic reactions, or autoimmune flares. So in theory, Picrorhiza could push against what your steroid is trying to do, and you might notice your symptoms (swelling, rashes, joint pain, or whatever you're treating) not staying as controlled.

I want to be honest about the strength of this concern. It comes from lab and animal work, not real human cases, so it's more of a theoretical caution than a proven problem. Still, if you take hydrocortisone for something serious like adrenal insufficiency or lupus, even a small dip in effect matters, so please run it by your pharmacist or doctor before adding Picrorhiza.

What Is Hydrocortisone Used For?

Brand names: Cortef, Hydrocortone, Carmol HC cream, Hydrocortisone Iodoquinol Cream, Vytone Cream, Massengil Medicated Towelettes, Dioderm, Efcortelan Cream, Mildison, Alphaderm, Calmurid HC, Efcortelan Ointment, Texacort, Alkindi Sprinkle

Hydrocortisone is used to calm skin inflammation and relieve itching caused by conditions that respond to corticosteroid treatment. In adults, it treats a broad range of inflammatory skin conditions. In children as young as 3 months old, it is specifically approved for mild to moderate atopic dermatitis (eczema).

It is also used in rectal foam form to treat inflammation of the lower bowel, such as ulcerative proctitis.
